Loma Linda, California - A new research study from Loma Linda University (LLU) shows that adding just a handful of pecans to your diet each day may inhibit unwanted oxidation of blood lipids, thus helping reduce the risk of heart disease. Researchers suggest that this positive effect was in part due to the pecan's significant content of vitamin E.

"Plant foods, including pecans, are rich sources of phytochemicals that can have a unique effect on the body," says LLU researcher Ella Haddad, DrPH, associate professor, department of nutrition, School of Public Health.

Pecans contain different forms of vitamin E - known as tocopherols - which protects fats from oxidation. Pecans are especially rich in one form of vitamin E - gamma tocopherol.

"We found that eating pecans increased levels of gamma tocopherol concentrations in the blood and subsequently reduced a marker of lipid oxidation," adds Dr. Haddad.

Oxidation of fats in the blood - a process akin to rusting - is detrimental to health. When the "bad" cholesterol becomes oxidized, it is more likely to build up and result in arteriosclerosis.

These latest research findings on pecan's healthfulness were published in the latest issue of Nutrition Research, just released this week. They are from the second phase of a research project designed to evaluate the health benefits of pecans, according to Dr. Haddad. She analyzed blood samples from study participants (a total of 23 men and women between the ages of 25 and 55) who ate two diets: one that contained pecans and one that did not. Participants were randomly placed on either the American Heart Association's Step I diet or a pecan-enriched version of the Step I diet. (The pecan-enriched diet was similar to the Step I diet but replaced 20 percent of calories with pecans). After four weeks on one diet, they then switched to the other diet.

In the laboratory analysis of blood samples from the research subjects, Dr. Haddad's team found that the pecan-enriched diets significantly reduced lipid oxidation (by 7.4 percent) versus the Step I diet. Oxidation levels were evaluated using the TBARS test, which measures oxidation products. The researchers also found that blood levels of tocopherols were higher after participants were on the pecan diet. Cholesterol-adjusted plasma gamma-tocopherol in the study participants' blood samples increased by 10.1 percent (P < .001) after eating the pecan diet. The researchers concluded that these data provide some evidence for potential protective effects of pecan consumption in healthy individuals.

Another key research finding, beyond the reduced level of blood lipid oxidation, was that the various phytochemicals found in pecans seem to be protective of the pecan's high levels of unsaturated fat. All unsaturated fats in foods can be prone to oxidation themselves (which some may describe in foods as rancidity). So, did eating pecans lead to an increased risk of oxidation? No, according to this analysis, which found that pecans, while high in unsaturated fat, are "self-protective" due to their vitamin E content (tocopherols) and relatively high content of complex phytonutrients, some of which have been identified as proanthocyanidins, or condensed tannins, which are recognized for their ability to slow the oxidation process.

"We concluded that even though the pecan diet was high in unsaturated fats, which one may think would increase blood oxidation, that did not happen. We found the opposite result: the pecan diet showed reduced oxidation of blood lipids," states Dr. Haddad.

The dramatic initial research findings from this research project were published earlier in The Journal of Nutrition by LLU's Joan Sabate, MD, DrPH, professor and chair, department of nutrition, School of Public Health. He found that the pecan-enriched diet lowered levels of LDL cholesterol by 16.5 percent - more than twice as much as the Step I diet. Similarly, the pecan-enriched diet lowered total cholesterol levels by 11.3 percent (also twice as much as the Step I diet).

In one of the most rigorous studies ever conducted to determine how well people comprehend the information provided on food nutrition labels, researchers have found that the reading and math skills of a significa nt number of people may not be sufficient to extract the needed information, according to an article published in the November issue of the American Journal of Preventive Medicine.

Using standardized and validated tests for literacy (REALM -Rapid Estimate of Adult Literacy in Medicine) and numeracy (WRAT3 - Wide Range Achievement Test), researchers from Vanderbilt University Medical Center surveyed 200 primary care patients from a wide socioeconomic range. A Nutrition Label Survey (NLS), designed with input from registered dietitians, primary care providers, and experts in health literacy/numeracy to evaluate patient understanding of current nutrition labels, was used to measure comprehension of current food nutrition labels. One part of the NLS asked subjects to interpret food labels, such as determining carbohydrate or caloric content of an amount of food consumed. The other part asked patients to choose which of two foods had more or less of a certain nutrient, giving patients a 50/50 chance to guess the correct food item. Also, half of the survey questions involved products that were clearly labeled on their package as "reduced carb," "low carb," or designed for "a low-carb diet."

Sixty-eight percent of patients had at least some college education, and 77% had at least 9th-grade level literacy skills. However, 63% of patients had less than 9th-grade numeracy skills. Over 40% had a chronic illness for which specific dietary intervention is important (e.g., hypertension, diabetes), and 23% reported being on a specific diet plan. Most patients reported using food labels and found labels easy to understand.

Overall, patients correctly answered 69% (SD 21%) of the NLS questions. For example, only 32% of patients could correctly calculate the amount of carbohydrates consumed in a 20-ounce bottle of soda that had 2.5 servings in the bottle. Only 60% of patients could calculate the number of carbohydrates consumed if they ate half a bagel, when the serving size was a whole bagel. Only 22% of patients could determine the amount of net carbohydrates in 2 slices of low-carb bread, and only 23% could determine the amount of net carbohydrates in a serving of low-carb spaghetti. Common reasons for incorrect responses included misapplication of the serving size, confusion by extraneous material on the food label, and incorrect calculations.

According to Russell L. Rothman, MD MPP, "The study showed that many patients struggle to understand current food labels, and that this can be particularly challenging for patients with poor literacy and numeracy (math) skills. Poor understanding of nutrition labels can make it difficult for patients to follow a good diet. Of particular concern are situations that involve interpretation and application of serving size. There are many opportunities for health care providers to improve how they talk to patients about using food labels and following diets. There are also opportunities for the FDA to improve how food labels are designed in order to improve how patients take care of their nutrition.

For those who can find no time for exercise may have just run out of excuses. A study published in the Journal of Physiology last week said that interval training can be as effective as traditional training metho ds.

Interval training involves short bursts of intense training followed by times for recovery. That kind of training is used in sports such as running and cycling.

The study, conducted by a team of researchers at Ontario's McMaster University, involved 16 college-aged students which were divided into two groups. They were then made to follow different fitness regimens for two weeks. One group did interval training while the other group followed traditional training methods.

After two weeks, the group that did interval training spent only a total of 2.5 hours doing exercise while the other group spent a total of 10.5 hours. But in spite of the difference in the time spent in training, the groups’ improvement in terms of exercise performance and endurance were the same.

Walking is a popular form of exercise, but may not be enough to experience significant health benefits, a University of Alberta study shows.

"Generally, low-intensity activity such as walking alone i
s not likely going to give anybody marked health benefits compared to programs that occasionally elevate the intensity," said Dr. Vicki Harber, lead author on the Health First study, which was presented recently at the American College of Sports Medicine annual conference.

Dr. Harber and her colleagues, Dr. Wendy Rodgers, Dr. Gordon Bell and Dr. Kerry Courneya of the Faculty of Physical Education and Recreation at the University of Alberta in Edmonton, Canada, were concerned that while people with health issues are encouraged to increase their volume of activity such as walking, there didn't seem to be much focus on the effort that needed to go into the activity.

The University of Alberta study put the popularized pedometer-friendly 10,000-step exercise program to the test against a traditional fitness program which incorporated cardio-based activities on equipment such as treadmills and stationary bicycles. The traditional group was asked to complete exercise at a moderate intensity, a level allowing for one or two sentences of conversation with ease. Intensity was not set for the walking group; they completed their daily exercise at a self-selected pace.

"When we matched the two programs for energy expenditure, we found that the traditional fitness program improved aerobic fitness and reduced systolic blood pressure, more than the 10,000-step lifestyle program," Dr. Harber said. Of the 128 sedentary men and women who completed the six-month research program, those who took part in a more active traditional fitness regimen increased their peak oxygen uptake, an indicator of aerobic fitness, by 10 per cent. Those who took part in the walking program experienced a four per cent increase. Systolic blood pressure also dropped by 10 per cent for the traditional fitness group, compared to four per cent for the group who just walked.

Other markers of overall health, such as fasting plasma glucose levels, response to a two-hour glucose tolerance test and various blood lipids were unaffected by either exercise program.

"Our concern is that people might think what matters most is the total number of daily steps accumulated, and not pay much attention to the pace or effort invested in taking those steps," Dr. Harber said. "The 10,000-step or pedometer-based walking programs are great for people--they are motivating, and provide an excellent starting point for beginning an activity program, but to increase the effectiveness, one must add some intensity or "huff and puff" to their exercise. Across your day, while you are achieving those 10,000 steps, take 200 to 400 of them at a brisker pace."

"You've got to do more than light exercise and move towards the inclusion of regular moderate activity, and don't be shy to interject an occasional period of time at the vigorous level."

Women who are physically active but have lower aerobic fitness and poorer overall fitness may be at greater risk for stress fractures and other overuse injuries in the lower extremity. This is according to the ar ticle posted yesterday at www.medicalnewstoday.com.

The report is based on the study conducted by the Indiana-based, American College of Sports Medicine (ACSM). Aside from the connection between poor overall fitness to overuse injuries, the researchers at the ACSM also said that menstrual irregularities can contribute to lower-extremity stress fracture.
